At Community Care Homes care is based around each persons' individual needs. Whatever the need, care is delivered with patience, kindness and understanding.

Our aim is to help those who come to live with us to remain in control of their own lives, as far as possible, and thus to maintain their personhood.

Person centered care is the term for this. Our philosophy is simple - our residents should still feel as if they are living at home with additional support and care.
 
Short-term Respite Care PDF Print E-mail

We offer short-term respite care in our homes for people who are usually looked after by a relative or carer at home or have been in hospital and require convalescence care following discharge.

Respite stays give home based carers the opportunity to take scheduled holidays or breaks throughout the year. Carers can relax with peace of mind knowing that their relative or patient is in a safe and homely environment with 24-hour care available.

We are committed to helping build links between the public and independent sector and we continue to seek ways to develop intermediate care services and improve the opportunities for rehabilitation of service users.

In our view, intermediate care is an intensive short-term service that is about promoting independence, so that people avoid admission to hospital, or can return to their own home after rehabilitation.

Community Care Homes support the NHS End of Life Care philosophy.

The compassionate approach that treats, comforts and supports individuals to enable them to live as well as possible until they die. It includes physical care, management of pain and other symptoms; the provision of psychological, social, spiritual and practical support is also important and it encompasses support for families and friends up to and including the period after death.

Community Care Homes offer Dementia Services and are also sometimes referred to as homes offering care for those who are elderly mentally infirm.

They offer a safe and secure environment for all of our residents and are designed in a way that helps residents to feel settled and aware of their environment, whatever the type and level of dementia they suffer from.
